新生兒腹瀉是由多種病原多種因素引起的,以大便次數(shù)增多和大便性狀改變?yōu)樘攸c(diǎn)的一組消化道綜合征【1】。主要是由于新生兒消化系統(tǒng)發(fā)育不成熟,生長(zhǎng)發(fā)育快,機(jī)體防御功能差,腸道菌群失調(diào)等因素造成的【2】。因此,在積極治療疾病的同時(shí),給予積極完善的護(hù)理干預(yù)措施可以促進(jìn)患兒的康復(fù)。對(duì)我兒科2013年1月-2013年12月收治的新生兒腹瀉患兒給予護(hù)理干預(yù)措施,取得滿(mǎn)意效果,現(xiàn)將護(hù)理干預(yù)措施報(bào)道如下。
1 資料與方法
1.1 一般資料
選取2013年1月-2013年12月在我兒科治療的新生兒腹瀉患兒240例作為研究對(duì)象,隨機(jī)分成對(duì)照組和觀察組,對(duì)照組120例,其中男78例,女42例;年齡6d-24d,平均年齡14.1±0.6d;腹瀉次數(shù)5-14次,每次45-190ml。觀察組120例,其中男74例,女46例;年齡11d-23d,平均年齡15.3±0.6d;腹瀉次數(shù)4-15次,每次40-180ml。所有患兒入院后均經(jīng)大便常規(guī)檢查顯示,白細(xì)胞(++-+++),脂肪球(+-++),均符合新生兒腹瀉的診斷標(biāo)準(zhǔn)【3】。所有患兒家屬均簽署知情同意書(shū)。兩組患兒在年齡、性別、腹瀉次數(shù)方面無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05),具有可比性。

要根據(jù)患兒的腹瀉、嘔吐、食欲和消化情況,確定飲食的方法,對(duì)嘔吐、腹瀉嚴(yán)重患兒可暫時(shí)禁食6-8h,以利腸道休息,減少?lài)I吐及腹瀉次數(shù),一般不禁水,行母乳喂養(yǎng)的新生兒應(yīng)持續(xù)喂哺母乳;對(duì)于人工喂養(yǎng)的腹瀉較重的新生兒,要減少奶量并用水稀釋?zhuān)捎?份牛乳加1/2份水,待腹瀉好轉(zhuǎn)后逐漸增加至全奶。當(dāng)患兒腹瀉好轉(zhuǎn)時(shí),可以按照從稀到稠、從軟到硬的規(guī)律逐漸過(guò)渡的原則,少量多餐。
1.2.2.3 健康教育
首先宣傳母乳喂養(yǎng)的優(yōu)點(diǎn),避免在夏季斷乳;新生兒的奶瓶用前應(yīng)煮沸消毒20分鐘,吃剩的奶液應(yīng)倒掉;母親應(yīng)保持乳房的清潔,勤換內(nèi)衣,盡量減少感染的機(jī)會(huì);對(duì)于母乳不足可采取混合喂養(yǎng)及人工喂養(yǎng)的辦法,但是要注意飲食調(diào)配,避免發(fā)生碳水化合物消化不良,影響寶寶生長(zhǎng)發(fā)育;同時(shí)注意新生兒腹部的保暖,避免受涼。

1.1資料
本院2011年11月~2013年11月診治的急性左心衰患者142例,年齡為47~81歲,平均年齡為67.3±5.9歲,其中男性108例,女性34例。患者均符合急性左心衰的診斷標(biāo)準(zhǔn),且經(jīng)臨床表現(xiàn)和實(shí)驗(yàn)室檢查確診,排除患有其他器質(zhì)性病變、血液病、免疫性疾病的患者,均行無(wú)創(chuàng)機(jī)械通氣治療。根據(jù)護(hù)理方案分為兩組,常規(guī)護(hù)理患者71例為對(duì)照組,護(hù)理干預(yù)患者71例為觀察組,兩組患者的基礎(chǔ)資料(性別、年齡等)差異均無(wú)統(tǒng)計(jì)學(xué)意義(p>0.05),具有可比性。
1.2方法
觀察組患者實(shí)施護(hù)理干預(yù):⑴基礎(chǔ)干預(yù)。護(hù)理人員采用和藹可親的語(yǔ)言和患者進(jìn)行交流,建立良好的醫(yī)患關(guān)系,取得患者的信任,通過(guò)溝通與交流,幫助患者消除焦慮、抑郁、恐懼、害怕、緊張、擔(dān)心等不良心理因素,幫助患者建立配合治療的信心和決心,建立良好的干預(yù)基礎(chǔ)。⑵情緒干預(yù)。向患者講解疾病的相關(guān)知識(shí)和治療的有效方法,使患者了解到疾病治療的目的和意義,使其可以正視疾病的治療,在治療過(guò)程中,患者出現(xiàn)恐懼感時(shí)要給予及時(shí)的安慰和鼓勵(lì),采用支持性語(yǔ)言和肢體語(yǔ)言穩(wěn)定患者的情緒,提高患者的耐受性,消除患者的不安狀況,減輕患者的恐懼病癥。⑶行為干預(yù)。指導(dǎo)患者在治療過(guò)程中采用端坐位,將頭部與肩頸處于同一水平位置,將頭后仰,確保呼吸道通暢,佩戴合適的面部口鼻罩,調(diào)節(jié)松緊度,設(shè)定吸氣相壓力由6cmH2O開(kāi)始,設(shè)定呼氣相壓力由2cmH2O開(kāi)始,增到患者的耐受,要確?;颊叩拿娌科つw無(wú)受損,并在皮膚接觸部位放置紗布,減輕患者的壓迫感,在皮膚接觸部位涂抹凡士林,3h更換1次,每次20min,給予患者局部皮膚按摩等。⑷病情監(jiān)護(hù)。注意觀察患者的呼吸節(jié)律和心率狀況,確保主動(dòng)呼吸和呼吸機(jī)同步,注意保持患者的呼吸道通暢,在加濕器內(nèi)添加蒸餾水,將氣體濕化后進(jìn)入氣道,指導(dǎo)患者掌握正確的呼吸方法。對(duì)照組患者實(shí)施常規(guī)護(hù)理?;颊咴谥委熯^(guò)程中,給予患者常規(guī)的基礎(chǔ)護(hù)理,保證患者的病房通風(fēng)良好和環(huán)境的整潔衛(wèi)生,及時(shí)更換患者的床單被褥,只要患者實(shí)施正確的飲食和作息等。
1.3評(píng)定標(biāo)準(zhǔn)
主訴恐懼情況的評(píng)定標(biāo)準(zhǔn):⑴輕度。患者不回避治療,無(wú)明顯的恐懼表現(xiàn)。⑵中度。患者有恐懼表現(xiàn),曾試圖回避治療。⑶重度?;颊哂袕?qiáng)烈的恐懼表現(xiàn),盡力回避治療,在護(hù)士幫助下才能完成治療。
主訴配合情況的評(píng)定標(biāo)準(zhǔn):⑴積極?;颊呖梢苑e極主動(dòng)配合治療。⑵一般。患者可以基本配合治療。⑶較差?;颊咝枰谧o(hù)士幫助下勉強(qiáng)完成治療。
主訴并發(fā)癥情況的評(píng)定標(biāo)準(zhǔn):⑴輕度?;颊呙娌渴軌禾幤つw發(fā)紅和疼痛,停機(jī)5~10min后可恢復(fù)正常。⑵中度。患者主訴有腹脹感,可耐受,面部受壓處皮膚發(fā)紅和疼痛,停機(jī)5~10min不能恢復(fù)正常。⑶重度?;颊咧髟V腹脹明顯,行胃腸減壓和肛管排氣后恢復(fù),面部受壓處皮膚破損。

臨床上,輪狀病毒感染性腹瀉在兒科中具有較高的發(fā)病率,引發(fā)本病的病原體主要為輪狀病毒,此病的主要臨床癥狀表現(xiàn)除了腹瀉之外同時(shí)普遍并發(fā)有脫水、發(fā)熱等癥狀,嚴(yán)重的甚至出現(xiàn)代謝紊亂以及酸中毒,通常好發(fā)于秋冬季節(jié),若無(wú)法及時(shí)診斷并接受針對(duì)性治療,嚴(yán)重的甚至可直接引發(fā)死亡[1]?,F(xiàn)階段,臨床上對(duì)本病的發(fā)病機(jī)制尚無(wú)法明確,因而無(wú)統(tǒng)一的且具有明顯效果的治療標(biāo)準(zhǔn)[2]。為此,我院引入西咪替丁與干擾素聯(lián)合微量元素鋅治療,現(xiàn)將療效報(bào)道如下。
1 資料與方法
1.1 一般資料
67例均為我院自2015年11月~2016年12月期間收治的輪狀病毒感染性腹瀉患兒,納入標(biāo)準(zhǔn):臨床癥狀均符合本病的癥狀表現(xiàn),同時(shí)經(jīng)病原學(xué)檢查確診。經(jīng)醫(yī)院倫理委員會(huì)審批后,將67例患兒隨機(jī)分為治療組與對(duì)照組兩組,對(duì)照組32例,其中男17例,女15例;年齡0.7~4.5歲,平均年齡(1.6±0.8)歲。治療組35例,其中男19例,女16例;年齡0.8~4.4歲,平均年齡(1.6±0.7)歲。兩組患兒在基線(xiàn)資料方面對(duì)比差異無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05),具有可比性。
1.2 方法
兩組均給予常規(guī)檢查,同時(shí)結(jié)合脫水的嚴(yán)重程度給予補(bǔ)液治療,給予發(fā)熱以及嘔吐者對(duì)癥治療,注意兩組均不能給予抗生素治療。基于此,對(duì)照組采用西咪替丁注射液(湖南漢森制藥股份有限公司;H43021799)10~15mg/(kg?d)治療,給藥方式為靜脈滴注,同時(shí)增加重組人干擾素α1b [安徽安科生物工程(集團(tuán))股份有限公司;S20040010]治療,給藥方式為注射,劑量為100萬(wàn)U/次,1次/d。在此基礎(chǔ)上,治療組聯(lián)合葡萄糖酸鋅口服液(湖北福人金身藥業(yè)有限公司;H20065022)治療,給藥方式為口服,每支的含鋅量為0.03g,1支/d,兩組的療程均為10d。
1.3 觀察指標(biāo)及評(píng)定標(biāo)準(zhǔn)
記錄兩組大便及體溫恢復(fù)時(shí)間、脫水糾正時(shí)間。記錄兩組面色潮紅、煩躁以及腹瀉加重等不良反應(yīng)情況。對(duì)兩組治療效果進(jìn)行評(píng)估,若3d后,癥狀消失,同時(shí)糞便次數(shù)以及形狀均正常則評(píng)定為顯效;若3d后癥狀明顯緩解,同時(shí)糞便次數(shù)及形狀明顯好轉(zhuǎn)則評(píng)定為有效 ;若3d后癥狀未見(jiàn)任何緩解,同時(shí)糞便次數(shù)及形狀未見(jiàn)任何改善則評(píng)定為無(wú)效。總有效率=100%-無(wú)效率。
1.4 統(tǒng)計(jì)學(xué)分析
使用SPSS19.0統(tǒng)計(jì)學(xué)軟件處理數(shù)據(jù),兩組計(jì)數(shù)資料對(duì)比行χ2檢驗(yàn),兩組計(jì)量資料對(duì)比行t檢驗(yàn),用P
2 結(jié)果
2.1 兩組治療基本情況比較
治療組脫水、大便以及體溫等的恢復(fù)時(shí)間均分別明顯短于對(duì)照組(P
2.2 兩組治療效果比較
治療組治療總有效率為97.14%,明顯高于對(duì)照組的71.88%(P
2.3 兩組不良反應(yīng)比較
治療組不良反應(yīng)發(fā)生率為2.86%,與對(duì)照組的不良反應(yīng)發(fā)生率6.26%對(duì)比差異無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05),詳見(jiàn)表3。
3 討論
臨床上,感染性腹瀉屬于兒科中常見(jiàn)的一種腸道感染性疾病,出現(xiàn)腹部疼痛以及糞便不成形等為本病的主要癥狀表現(xiàn)[3-4]。人體小腸內(nèi)部的載膜絨毛上皮細(xì)胞中存在一種可以與輪狀病毒外殼中含有的VP4相互結(jié)合的病毒原體,因此小腸成為了輪狀病毒侵入的重要部位[5]。待病毒繼續(xù)侵入機(jī)體上皮細(xì)胞之后,便會(huì)誘發(fā)機(jī)體內(nèi)的大量非結(jié)構(gòu)蛋白糖基化,進(jìn)而引發(fā)細(xì)胞膜發(fā)生紊亂現(xiàn)象,例如鈣離子通透性,一旦機(jī)體細(xì)胞內(nèi)的鈣離子出現(xiàn)大量增多的現(xiàn)象,便會(huì)引發(fā)高鈣低氯癥狀,進(jìn)一步可增加病毒非結(jié)構(gòu)蛋白NSP3抑制蛋白正常合成的風(fēng)險(xiǎn),進(jìn)而將影響小腸上皮細(xì)胞的運(yùn)行,嚴(yán)重的甚至可導(dǎo)致其脫落,極大的減少了雙糖轉(zhuǎn)化為單糖過(guò)程中所需的腸載膜上絨毛酶的數(shù)量,隨著時(shí)間的延長(zhǎng),雙糖大量堆積在腸道內(nèi),便極易增加腸腔內(nèi)滲透壓,進(jìn)而誘發(fā)腹瀉、嘔吐及脫水等癥狀,嚴(yán)重的可并發(fā)水電解質(zhì)紊亂以及酸中毒等嚴(yán)重威脅患兒生命的癥狀[6-7]。
西咪替丁是一種H2受體拮抗劑,能夠顯著抑制因組胺或者胃泌素引發(fā)的胃酸分泌,進(jìn)而避免受損腸黏膜長(zhǎng)期處在酸性環(huán)境中而對(duì)其起到保護(hù)作用,并有利于為絨毛的修復(fù)創(chuàng)造良好條件[8-9]。此外,此種藥物還具有免疫調(diào)節(jié)作用,能夠通過(guò)提高患兒機(jī)體內(nèi)的T淋巴細(xì)胞的轉(zhuǎn)化率而起到增強(qiáng)其免疫力的效果,進(jìn)而能夠避免炎癥反應(yīng)誘發(fā)的一系列繼發(fā)性損傷問(wèn)題[10-11]。干擾素則具有免疫調(diào)節(jié)以及抗病毒效應(yīng)等多種生理功能,在本病的治療過(guò)程中具有顯著的價(jià)值[12]。刷狀緣中大部分酶類(lèi)均對(duì)鋅元素存在較強(qiáng)的依賴(lài)性,同時(shí)鋅離子能夠顯著的抑制小腸黏膜的增生,且能夠促進(jìn)其完全角化,進(jìn)而能夠?qū)δc黏膜起到保護(hù)作用,此外,還可抑制胃腸激素分泌,增加腸內(nèi)鈉氯離子的吸收能力,進(jìn)而能夠?qū)δc腔的滲透壓起到降低的效果[13]。除此之外,鋅離子還具有免疫調(diào)節(jié)以及抑菌等多種功效。特別值得關(guān)注的是,我院在采用西咪替丁與干擾素常規(guī)治療的基礎(chǔ)上增加微量元素鋅的一個(gè)理論基礎(chǔ)是此類(lèi)型的患兒血清中鋅代謝有異常的現(xiàn)象[14-16]。
本研究中,治療組脫水、大便以及體溫等的的恢復(fù)時(shí)間均分別明顯短于對(duì)照組(P
綜上,將西咪替丁與干擾素聯(lián)合微量元素鋅應(yīng)用于輪狀病毒感染性腹瀉患兒的治療中,可以快速消除患兒的癥狀,促進(jìn)其康復(fù),具有療效理想,安全性高的特點(diǎn),值得臨床上推廣及應(yīng)用。
